Catholic Charities West Virginia is seeking a Hospital Transition Program Service Specialist in Wheeling to coordinate post-discharge services and on-site food pantry operations. • Must have transportation for travel as needed.

Aufgaben

• Work with Northern Region Director and CPO to develop Hospital Transition Program.
• Coordinate screening and referral for at-risk patients pre-discharge.
• Communicate with clients before discharge and assess needs.
• Coordinate on-site pantry operations and recruit volunteers.
• Conduct home visits and provide direct care toward goals.
• Document assessments and progress according to guidelines.
• Ensure grant requirements are met and documented.
• Build relationships with Wheeling Hospital, parish, and faith-based partners.
• Foster parish involvement and supervise volunteers.
• Represent CCWVa and the program to community stakeholders.
