Job summary

Dartmouth Health Hampstead Hospital is seeking a Social Worker to provide therapeutic and case management services, collaborate with aftercare providers, and coordinate patient transitions. The role includes individual, family, and group therapy, treatment planning, and psychosocial formulation.

Required licensure includes LICSW pathway; BLS and CPI certifications are listed. Pay range is $25.82–$40.03 per hour, with 40 hours per week, day shift, and 1.00 FTE.

Qualifications

• Master’s degree in social work or related field from an accredited institution
• Internship meeting professional standards and LICSW licensure path likely required

Responsibilities

• Provides direct and indirect social work services to patients and families, including counseling, case management, psychosocial assessments, aftercare planning and family meetings
• Coordinates discharge planning and continuing care plans with treatment team recommendations
• Provides clinical consultations to treatment team members and may train staff on program policies and evidence-based interventions
• Develops relationships with community healthcare providers and consults collateral outpatient providers, residential programs, and schools
• Performs level-of-care assessments and pre-certification reviews with insurance companies
• Completes and reviews documentation to Joint Commission, Medicaid and Medicare standards; LICSWs may provide supervision and mentoring
• Promotes patient safety culture and participates in safety training and Performance Improvement activities
