Job summary

Advocate Health is seeking a licensed social worker to join the Case Management team at Advocate South Suburban Hospital. This full-time role focuses on psychosocial assessment, discharge planning, and care coordination across the continuum.

The position requires IL-LSW eligibility with a CSWE-accredited MSW, at least 1 year of healthcare social work experience, and availability for Monday–Friday shifts with weekends and holidays as needed. Benefits are provided.

Qualifications

• MSW from CSWE-accredited program.
• IL-LSW eligibility; LCSW preferred.
• At least 1 year of social work experience in healthcare.
• Knowledge of social work principles and methods in a health-care setting.

Responsibilities

• Provide social work services including psychosocial assessment, discharge planning and follow-up.
• Collaborate with multidisciplinary team to coordinate post-acute placement and community resources.
• Educate patients and families about advance directives and end-of-life issues where appropriate.
• Document activities and ensure compliance with department protocols and standards.
• Support care coordination across the continuum to optimize LOS and avoid readmissions.
• Assist with legal proceedings as needed to appoint decision-makers.
