Job summary

Delray Medical Center, part of Tenet Healthcare, is seeking a Per Diem Social Worker with an MSW to assess transition needs and coordinate discharge planning. You will support patients and families, ensure resources are aligned with care plans, and collaborate with a multidisciplinary team.

The role emphasizes complex psychosocial assessments, readmission prevention, and timely throughput while maintaining high-quality patient care and compliance with standards.

Qualifications

• Master in Social Work (MSW) is required or strongly preferred.
• Active license to practice in the state(s) covered.
• ACM (Accredited Case Manager) is preferred.
• Two years acute hospital experience preferred.

Responsibilities

• Perform complex psycho-social assessments and interventions to promote timely discharge and prevent readmissions.
• Coordinate transition planning including care coordination across providers.
• Educate physicians, patients, families, and staff on care plans and resources.
• Lead multidisciplinary care conferences and facilitate referrals to other departments.
