Job summary

The Christ Hospital is seeking a licensed social worker (MSW required) to assess psychosocial needs, develop treatment plans, and coordinate care for patients and families. You will collaborate with physicians and mental health professionals to promote optimal outcomes and connect patients with community resources.

Responsibilities include crisis management, advocacy, and documentation, with a focus on ethical practice and maintaining patient safety in a hospital setting.

Qualifications

• Exceptional skill interviewing patients and families in crisis.
• Ability to prioritize many simultaneous demands.
• Medical and psychiatric competence including knowledge of health policy, regulations, and legislation and community resources.
• Self-awareness, professionalism, and good judgment in handling emotional and confidential issues.

Responsibilities

• Provide psychosocial counseling and therapeutic interventions for patient/family.
• Educate and manage crises for patient and family.
• Facilitate healthcare decision-making.
• Identify psychosocial barriers to treatment and advocate for resources.
• Collaborate with interdisciplinary team to review plans and ensure continuity of care.
• Document interventions clearly and timely in charting and reporting metrics.
