Job summary

Qualifications

• Masters’ Degree in Social Work required.
• Two years hospital or community social work experience with case management.
• Knowledge of medical and social work terminology.
• Ability to use standard office equipment (computer, printer, fax, etc.).

Responsibilities

• Complete bio-psychological assessments and develop post-discharge care plans.
• Coordinate referrals to home health, DME, dialysis, rehab, and nursing facilities.
• Serve as a resource for staff, patients and families regarding community agencies.
• Manage adoptions and assist with special programs and education.
