Jefferson Health Northeast is seeking a dedicated Social Worker to assess and support patients and families, develop discharge plans, and collaborate with a multidisciplinary team. The role requires an MSW and hospital-based social work experience, with responsibilities spanning crisis intervention, advocacy, and coordination of care with external resources and placement

Qualifikationen

• MSW degree from an accredited program.
• 2 years of hospital-based social work experience.
• Knowledge of medical terminology and disease processes; community resources; insurance regulations.

Aufgaben

• Conduct psychosocial assessment and reassignment of patients/families related to social, emotional, and cognitive needs.
• Develop and implement discharge plans in collaboration with the multidisciplinary team.
• Provide counseling for patients/families to assist with coping with diagnoses, chronic illness, and end-of-life care.
• Advocate for patients to obtain necessary services and coordinate with community resources.
• Coordinate team and family conferences to facilitate patient care progression and discharge planning.
• Document updates in the EMR and ensure completion of placement documentation and referrals as needed.
