Job summary

Magee Rehabilitation Hospital is seeking a social worker to provide psychosocial assessments, discharge planning, and counseling to patients and families. You will work with the multidisciplinary team to develop and implement interventions addressing social, emotional, and cognitive needs related to illness and discharge.

The role requires an MSW with hospital-based experience and a focus on advocacy to ensure patients obtain necessary services.

Qualifications

• Master’s Degree in Social Work from an accredited program is required.
• 2 years hospital-based social work experience required.
• Experience with discharge planning and counseling of patients/families is preferred.

Responsibilities

• Provide psychosocial assessment and address social, emotional, and cognitive needs.
• Develop and implement intervention plans related to identified needs.
• Collaborate with multidisciplinary team to ensure appropriate discharge planning.
• Provide counseling to patients and families to assist with coping with illness and diagnoses.
• Advocate for patients and families to obtain needed services.
