Job summary

Qualifications

• Master's degree in social work.
• Preferred California BBS Licensed Clinical Social Worker (LCSW).

Responsibilities

• Provide consultation, and psychosocial counseling and support services to the Sharp Health Care customer base.
• Mobilize patients and families' resources to receive maximum benefits of medical care and achieve optimal functioning.
• Assess psychosocial needs and complete psychosocial assessments.
• Communicate assessment findings per department guidelines and develop treatment plans with the team.
• Attend rounds, document progress, and coordinate patient care conferences.
