Qualifications

• Master’s Degree in Social Work from an accredited program (MSW).
• 2 years hospital-based social work experience.
• Knowledge of medical terminology and disease processes; community resources; insurance regulations.

Responsibilities

• Provide psychosocial counseling and crisis intervention to patients and families.
• Collaborate with multidisciplinary teams to establish safe discharge plans.
• Coordinate and facilitate family conferences and placement decisions.
• Advocate for patient/family access to needed services and resources.
