EvergreenHealth Monroe is seeking a Utilization Review professional to coordinate discharge planning and psychosocial assessments. You will work with the Clinical Care Transitions Manager to ensure safe, efficient patient transitions and address social, environmental, and financial factors affecting hospitalization.

Requires a Master’s Degree in Social Work and active licensure per the job’s preferences; 2 years in acute or community hospital settings; English proficiency; strong organizational

Qualifications

• Master’s Degree in Social Work or related field is required.
• Licensure such as LICSW/LASW/LSWAA is preferred.
• Two years of social work experience in an acute hospital or community setting is required.
• Excellent communication and customer service skills, with ability to work under pressure.

Responsibilities

• Provide psychosocial counseling to patients and families about illness-related adjustment.
• Coordinate discharge planning, including referrals to SNF, home care, and other resources.
• Collaborate with care teams to review discharge needs and options.
• Facilitate third-party payment processes for discharge planning issues.
