Job summary

TriStar Greenview Regional Hospital in Bowling Green, KY seeks a Social Worker (BSW/LBSW/LCSW) to promote patient-centered care, assess psychosocial needs, and coordinate discharge planning within Med/Surg. You will collaborate with the care team to address barriers to discharge and connect patients to community resources.

You will support patients and families in making difficult decisions while honoring their values and preferences in a fast-paced healthcare environment.

Qualifications

• Bachelor's degree in Social Work or related field.
• License-eligibility or licensure pathway (LBSW/LCSW/LMSW-RCSW-Intern) preferred.
• Experience in hospital or discharge planning is a plus.

Responsibilities

• Collaborate with RN case managers and care teams to identify patient needs.
• Assess psychosocial needs and develop discharge plans for high-risk patients.
• Coordinate meetings with patients, families, and caregivers.
• Provide education on community resources and post-acute care criteria.
• Reassess and modify discharge plans with the clinical team as needed.
