Job summary

Qualifications

• Master's in Social Work from an accredited program.
• Active Social Worker license in the state of practice.
• Experience with discharge planning and home health preferred.

Responsibilities

• Interviews patient and family to assess home environment and health history.
• Assists patients to understand and follow medical recommendations.
• Plans and coordinates discharge or post‑acute care at home.
• Arranges for transportation and placement into appropriate care settings.
• Maintains patient documentation in EMR.
• Supports department policies, safety, and quality assurance.
• Assists with data collection and reporting for hospital and care teams.
• Coordinates with case management for continuity of care.
• Provides individual, family, and group therapies as needed.
