Qualifikationen

• Master's in Social Work from an accredited program.

• Upon Hire: QMHP preferred in state of practice.

• Interviews patient and family to gather information on home environment, health history, and patient personality traits.
• Evaluates data to plan therapy aligned with medical treatment and social services.
• Assists patients and families to understand and follow medical recommendations to ensure compliance with care plans.
• Coordinates discharge planning and options for post‑acute care, including placement and transportation as needed.
• Prepares patient documentation in EMR and maintains department policies and quality standards.
