HCA HealthONE Swedish is seeking an experienced Social Worker III-Case Manager to assess psychosocial needs and coordinate interdisciplinary care for high-risk patients. The role supports discharge planning from admission and ongoing care decisions to optimize outcomes.

Requirements include an MSW from an accredited school and an active LCSW in Colorado, with 3–5 years of acute care hospital social work and case management experience preferred. Full-time, part-time, or PRN options available.

Formación

• Licensed Clinical Social Worker (LCSW) in CO required.
• Master of Social Work (MSW) from an accredited program required.
• 3–5 years of acute care hospital social work and case management preferred.

Responsabilidades

• Assess psychosocial, cultural, and financial needs impacting high-risk patients’ health and recovery.
• Coordinate services through an interdisciplinary process balancing patient/family needs and resources.
• Perform discharge planning from admission with ongoing reassessment.
• Apply crisis intervention and utilize system resources/referrals as needed.
