Job summary

Qualifications

• Master’s Degree in Social Work.
• Experience in discharge planning or hospital setting preferred.
• California BBS Licensed Clinical Social Worker (LCSW) preferred.
• Knowledge of EMR documentation and care coordination.

Responsibilities

• Provides psychosocial assessments and discharge planning.
• Collaborates with RN Case Manager/Transition Planner and interdisciplinary team.
• Communicates updates to the team regarding social needs affecting stay/discharge.
• Maintains social work credentials and adheres to policies and ethics.
• Manages referrals and connects patients with community resources.
• Escalates complex cases to department leadership when needed.
