Job summary

Qualifications

• Master Degree in Social Work from an accredited program.
• One year social work experience or one year social work internship.
• Kentucky CSW or Indiana LSW licensure.

Responsibilities

• Assess the patient’s continuum of care needs and ensure transition to the least restrictive environment.
• Collaborate with care team, care management team, patient and/or family to support psychosocial needs.
• Coordinate referrals for acute rehabilitation, home health, durable medical equipment and other resources as needed.
• Adhere to Norton Healthcare policies and apply professional expertise in delivering quality care.
