Job summary

Tucson Medical Center, through its TMCOne multispecialty offices, is seeking a Social Worker to interview patients and families, assess social needs related to medical problems, and coordinate support services.

You will liaise with the medical and nursing teams, arrange post-discharge referrals, assist in developing care plans, and help patients navigate environmental challenges to access benefits and community resources. A Master’s in Social Work and relevant clinical experience are required.

Qualifications

• Master’s degree in Social Work required for all departments.
• Hospice requires MSW and licensure (LCSW or LMSW preferred).
• Two years of social work experience in an acute care setting preferred.
• HeartSaver CPR/BLS certification required per department; AZ fingerprint clearance may be required.

Responsibilities

• Interviews patients and family members to obtain information about home environment, health history and patient needs.
• Plans and coordinates patient care and discharge planning with the care team.
• Acts as liaison between patient, family, hospital staff and outside agencies.
• Maintains accurate documentation in medical records.
