Job summary

Landmark Medical Center, a 214-bed acute care hospital in Woonsocket, RI, is seeking a Social Services II to provide emotional support, crisis intervention and counseling to Pediatrics, Adolescents, Adults and families. You will work as part of a multidisciplinary team to coordinate discharge planning and develop treatment plans.

Qualifications include a Master’s Degree in Social Work or a related field (or MS in Marriage and Family Therapy), licensure or registration as required, and at least

• Master’s Degree in Social Work or related field, OR Master’s Degree in Marriage and Family Therapy or related field.
• Current state license or registration within 3 months where licensure is not required; license required at time of hire if licensure exists.
• Minimum one year hospital or outpatient experience in mental health care.
• Experience with a diverse population with broad range of problems.
• Current state license required upon hire (LSW or higher).
• Preferred: Current BCLS (AHA) certificate upon hire.

Responsibilities

• Provide emotional support, crisis intervention and counseling for Pediatrics, Adolescents, Adults and families; information and referral; coordination of discharge planning.
• Complete case management & milieu management, develop and re-evaluate Treatment Plans; work with a multidisciplinary team; document and maintain clinical records.
• Coordinate discharge planning and ensure documentation is complete for each client during their stay.
