Job summary

Landmark Medical Center in Woonsocket, RI is seeking a Part Time Social Services II to join our 32-hour weekly day shift (8:00am–4:30pm), including weekends and holidays. You will provide emotional support, crisis intervention and counseling to patients and families as part of a multidisciplinary team.

Responsibilities include case and milieu management, psychosocial assessments, treatment plan development and discharge planning, with thorough documentation.

Qualifications

• Master’s Degree in Social Work or related field (LSW or higher preferred)
• Master’s Degree in Marriage and Family Therapy or related field
• Current state license required upon hire and maintained
• Minimum 1 year hospital/outpatient experience in mental health care
• Experience with diverse populations and complex psychosocial needs

Responsibilities

• Provide emotional support, crisis intervention and counseling for patients and families
• Perform case management and milieu management, develop and reevaluate treatment plans
• Coordinate discharge planning and document clinical records for each client
