Qualifications

• Requires PharmD or BS in Pharmacy prior to 2006.
• ASHP-accredited PGY1 or PGY2 residency may be required.
• 0–3 years of relevant experience.
• Current Indiana Pharmacist license or eligibility.
• Board certification or specialized training may be required.
• Basic Life Support (AHA) may be required.
• Travel to IU Health facilities may be required.
• Weekend rotations may be required.

Responsibilities

• Ensure safety and effectiveness of the medication use process.
• Process and dispense medications per orders or prescriptions.
• Medication therapy monitoring and outcomes assessment.
• Regulatory compliance and accountability for medications.
• Oversee pharmacist extenders and protect patient information.
• Provide medication information support to medical and nursing staff.
