Qualifications
- PharmD or BS in Pharmacy required; licensed or eligible in Indiana.
- ASHP-accredited PGY1/PGY2 residency preferred.
- 0–3 years of relevant pharmacy experience.
- Must be licensed or eligible for Indiana Pharmacist licensure; ability to travel may be required.
Responsibilities
- Provide comprehensive medication-related care to improve patient outcomes.
- Collaborate with healthcare team to ensure safe medication-use processes.
- Dispense medications per orders; monitor therapy for effectiveness and safety.
- Ensure regulatory compliance and maintain medication security and patient confidentiality.
- Oversee pharmacist staffing and contribute to care continuity; address medication-related issues.
