Job summary

Qualifications

• High School Diploma or GED required.
• Crisis intervention training desirable.
• Preference for involvement with a community organization or similar network.

Responsibilities

• Implements direct intervention activities for the Safe Streets Hospital Responder program and links injured individuals to Safe Streets and community resources.
• Meets patients and family members after violent injuries, coordinates referrals, and tracks services and action plans.
• Collaborates with staff to develop plans, maintains records, drafts reports, and completes documentation.
