SMG HSHS St. Mary’s Hospital Medical Center in Decatur, IL seeks a Licensed Physical Therapist to evaluate patients, develop and execute treatment plans, and lead therapy services within Rehab/Therapy/Sleep.

This full-time, day-shift role emphasizes education for patients and colleagues and coordination with interdisciplinary teams. Minimum requirements include graduation from an accredited program and state licensure.

Formación

• Graduate from an accredited program meeting licensure requirements in the state of practice.
• Licensed Physical Therapist in the state of practice is required.
• Certified BLS or completion within the first 90 days of employment is required.
• Illinois licensure within 1 year for individuals meeting all other applicable requirements.

Responsabilidades

• Evaluates all assigned patients and develops, implements, and executes on treatment plans as outlined by the plan of care.
• Delegates and directs discipline-specific therapy services and participates in team care.
• Educates patients, families, students, coworkers and self-professional advancement.
• Performs other professional duties related to care, therapy program services, and interdisciplinary teams.
