Job summary

St. Clair Hospital is seeking a Pharmacy Technician Medication History to support the multidisciplinary healthcare team.

You will collect comprehensive medication and allergy histories from patients and caregivers in the emergency department and inpatient units, review external records, and ensure accurate entry in the electronic medical record to guide treatment plans at admission and discharge. The role requires a high school diploma, PC proficiency, and either 3 years hospital tech

Qualifications

• High School diploma or equivalent.
• Fundamental PC skills and familiarity with Windows OS.
• 3 years hospital pharmacy technician experience or 2 years retail experience or 1 year medication history technician.
• Current Pharmacy Technician certification or eligible within 1 year of hire.
• Excellent verbal communication skills.
• Ability to triage workload independently during shift.
• Ability to solve problems independently in acute care settings.

Responsibilities

• Collect a complete and accurate medication and allergy history from patients and caregivers.
• Review records from nursing facilities and retail pharmacies as applicable.
• Enter updated medication lists into the patient’s medical record for use by prescribers.
• Assist with admission and discharge medication plans and orders.
