Job summary

Cardinal Health, Inc. is seeking a PRN Pharmacist to provide pharmaceutical care in a hospital setting. Responsibilities include interpretation of orders, preparation and dispensing of medications, and ensuring compliance with laws and documentation.

The role involves on-call duties and coordination with the pharmacy team to support patient safety and optimal therapy. The position requires a PharmD or Bachelor's degree in pharmacy, with 3+ years of hospital experience and active state licensure

Qualifications

• Bachelor's degree in pharmacy required; PharmD preferred.
• 3+ years related pharmacy experience.
• Currently licensed to practice in state of opening or eligible to reciprocity.
• In-patient hospital pharmacy experience strongly preferred.

Responsibilities

• Participate with on-call rotation.
• Reach facility within 90 minutes when on-call.
• Interpret and transcribe orders to patient profiles.
• Compound and dispense pharmaceuticals including sterile products.
• Issue controlled substances and maintain records.
• Monitor drug therapies for interactions and appropriateness.
• Support pharmacokinetics consult service and dosing per protocol.
• Review culture and susceptibility data for antibiotics.
• Detect and report adverse drug reactions.
• Promote formulary and rational drug therapy.
• Provide clinical consultation and clarification to practitioners.
• Document all clinical activities and interventions.
• Participate in quality improvement and medication use review activities.
• Contribute to orientation and training programs.
