Qualifications

Responsibilities

• Accurately prepares, verifies, and dispenses medications in accordance with prescription orders and regulatory standards.
• Reviews medication orders for appropriateness and optimizes therapy in collaboration with care team.
• Counsels patients and families on proper medication use, side effects, and adherence strategies.
• Collaborates with physicians, nurses, and pharmacists to resolve medication-related issues and improve outcomes.
• Maintains accurate documentation of interventions and patient consultations in the EHR.
• Participates in quality improvement and medication safety initiatives.
