St. Davids Round Rock Medical Center is seeking a dedicated Pharmacist to monitor, evaluate, and optimize pharmaceutical care for patients. You will interpret orders, dispense medications, and monitor therapy, ensuring safe, effective and cost-efficient treatment in a high-volume hospital setting.

The role requires a PharmD or B.S. in Pharmacy, Texas license, and sterile IV certification with preferred residency or board certification.

Formación

• B.S. in Pharmacy or PharmD required.
• Texas State Board of Pharmacy license current.
• Sterile Products (IV) Certification (ACPE) required.
• Board Certification in Pharmacotherapy or related field preferred.
• Hospital experience preferred.
• Post-graduate Residency training preferred.

Responsabilidades

• Interpret medication orders and transcribe to computer system.
• Provide medication therapy accurately and effectively.
• Accurately compound and dispense pharmaceuticals.
• Monitor drug therapy regimens for interactions, allergies and dose appropriateness.
• Suggest cost-effective therapeutic alternatives to medical staff.
• Support formulary stewardship and rational drug therapy selection.
• Assist with clinical services such as drug consults and anticoagulant monitoring.
• Complete notification reports for medication errors and adverse drug reactions.
