Job summary

Wooster Community Hospital is seeking a Pharmacy Medication Reconciliation Technician to accurately collect and enter patients' current medications into the hospital's EHR for physicians involved in admission and discharge. The role includes relaying issues to pharmacists or physicians and processing ED discharge prescriptions using the Retail Pharmacy Computer System.

The PMRT operates under the Director of Pharmacy/Pharmacy Clinical Manager and requires a high school diploma or GED with Ohio

Qualifications

High School diploma or GED required.
2–3 years of pharmacy technician experience; retail pharmacy technician experience preferred.
State of Ohio Board of Pharmacy Technician Certification required.

Responsibilities

Obtain a patient medication history and document accurately in the EHR with correct doses and schedules.
Process discharge prescriptions for ED and other areas using the Retail Pharmacy Computer System.
Obtain patient allergies and enter coded allergy information in the EHR.
Perform the monthly ED Unit Inspection and ensure medications are secured and within expiry dates.
Restock ED kits/crash cart drawers and exchange drug drawers after codes as needed.
Restock ED automated distribution cabinets and manage stock to avoid shortages.
Expedite delivery of time-critical medications during Stroke/ STEMI alerts and other emergencies.
