Job summary

Brown University Health seeks a Pharmacy History Technician to obtain and document home medication histories and allergies for admitted patients. This role supports the hospital-wide medication reconciliation process using interviews, outpatient pharmacy contacts, and EMR documentation to ensure accuracy.

The technician works in the Emergency or Admitting Departments, rotating day/evening shifts, and collaborates with pharmacists and nurses.

Qualifications

• High school graduate or equivalent.
• One to two years of college preferred.
• Maintains current RI Pharmacy Technician licensure; holds or is pursuing CPhT certification.

Responsibilities

• Obtains and compiles current home medication history lists including drug name, dose, form, frequency, and route.
• Gathers patient allergy information and documents in EMR.
• Communicates discrepancies to the provider/pharmacist and tracks follow-up.
• References multiple sources (home interviews, outpatient pharmacies, facilities) for accuracy.
• Ensures timely and accurate documentation in EMR and responds to providers and staff requests.
