Job summary

Brown University Health seeks a Pharmacy Medication History Technician to accurately obtain home medication histories and allergies for hospital admissions, supporting a progressive reconciliation program at The Miriam Hospital in Providence. You will interview patients and families, gather data from outpatient pharmacies, and document sources and findings in the EMR with precision.

Qualifications

• High school graduate or equivalent.
• Current Rhode Island Pharmacy Technician license.
• National Pharmacy Technician Certification (CPhT) within 12 months of hire.
• Prior outpatient pharmacy experience preferred.

Responsibilities

• Obtains and compiles current home medication history list and allergies.
• Documents detailed and verified information in the EMR.
• Communicates discrepancies identified to providers/pharmacists.
• Interacts professionally with patients, families, and healthcare team.
