Job summary

Engage Healthcare in Silver Spring, MD seeks a Clinical Liaison to lead the referral process for the local market. You will conduct timely clinical and financial evaluations of patient referrals and direct referrals to appropriate facilities.

You will develop solutions based on needs assessments to drive referrals, maintain relationships with hospital discharge planners, case managers, and facility staff, and support admissions with census and quality goals.

Qualifications

  • 3+ years of experience in long-term care or related industry.
  • 1–2 years sales/marketing experience is a plus; nursing background preferred.
  • Must understand long-term care including census goals and referral expectations.
  • Excellent verbal and written English communication skills.

Responsibilities

  • Generate referrals to support census and quality mix goals.
  • Build relationships with facilities and referral sources.
  • Respond promptly to inquiries from discharge planners and other referrals.
  • Conduct timely patient assessments after referral (goal: within 30 minutes).
  • Assist with pre-admission care planning as needed.
  • Monitor census, quality mix, and bed management needs across facilities.
  • Provide daily updates to Admissions & Marketing Directors on referrals and admissions.
  • Collaborate with Nursing Directors and Business Office Managers for admissions decisions.

Job description

Position: Clinical Liaison

Engage Healthcare is looking for someone to lead and coordinate the referral process for the Silver Spring area market. Conduct timely clinical and financial evaluations of patient referrals assessing the needs of patients and directing referrals to appropriate company facilities. Based on the needs assessments, the Marketing Liaison will develop solutions that meet identified needs, which will result in referrals to selected facilities.

Essential Duties & Responsibilities:
  • Generate referrals to selected facilities to support their census and quality mix goals.
  • Assist in building a relationship with facility and referral sources.
  • Respond timely to inquiry calls from hospital discharge planners and other referral sources.
  • Conduct patient assessments in a timely manner after a referral is given by a hospital discharge planner, case manager, social worker, physician, or family member. Note: Timely refers to assessments complete with a goal of 30 minutes after referral.
  • Assist as needed with pre-admission care planning.
  • Work with facility staff to understand census and quality mix goals and referral process expectations.
  • Understand census, quality mix and bed management needs of all facilities in assigned market.
  • Provide daily updates to the Admission & Marketing Directors regarding referral and admissions activities.
  • Maintain a close working relationship with the Directors of Nursing and the Business Office Managers to ensure appropriate and efficient decisions about prospective admissions.
  • Make admissions recommendations to facility management.
  • When admissions determinations are made, communicate with facilities about appropriate room and bed needs, as well as special needs of new admits to staff to facilitate a smooth transition
  • Maintain a working knowledge of Federal and State regulations and reimbursement (Managed Care, Medicare, and Medicaid).
  • Provide assistance with new Stakeholder orientation regarding the marketing and admissions process.
  • Maintain a strong working knowledge of managed care referral processes.
  • Other special projects and duties, as assigned.
Job Requirements:
  • Three (3) or more years of experience in long-term care or related industry.
  • One (1) to two (2) years sales and/or marketing experience a plus and case management experience preferred.Preference will be given to candidates with a nursing background.
  • Must understand long-term care business, including census and quality mix goals and referral expectations.
  • Effective verbal and written English communication skills.
  • Demonstrated intermediate to advanced skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation.
  • Highest level of professionalism with the ability to maintain confidentiality.
  • Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.
  • Customer service oriented with the ability to work well under pressure.
  • Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
