Base pay range

$70,000.00/yr - $85,000.00/yr

Avantara is hiring a motivated and innovative Hospital Liaison to oversee regional business development efforts and drive census growth for a group of Avantara locations. Based in Rapid City, South Dakota, this position emphasizes building partnerships with hospital discharge planners, creating community connections, and ensuring seamless transitions for residents across the region. This role would be a great fit for an LPN, RN, or someone with a patient care coordination background.

Responsibilities:

  • Cultivate and maintain relationships with referral sources, including hospital discharge planners, case managers, and social workers, across your assigned region.
  • Promote Avantara’s post-acute care services to healthcare providers, hospitals, and community organizations.
  • Work closely with facility leadership within your region to optimize admissions processes and enhance resident and family satisfaction.
  • Provide clinical onsite coordination with local hospital networks to ensure continuity of care during transitions.
  • Develop and execute strategic plans to increase census growth based on data-driven insights and market trends.
  • Represent Avantara at networking events, community engagements, and healthcare expos to strengthen brand presence.
  • Monitor referral pipelines and provide regular reports on activity, conversions, and growth metrics to support strategic goals.


Qualifications:

  • Hold a current, unencumbered LPN or RN nursing license in the state of South Dakota or patient care coodiantion/case management backgound. in the hospital or long-term care industry.
  • Successful experience in business development, hospital liaison, or healthcare sales, with a focus on relationship-building and strategic planning.
  • In-depth knowledge of post-acute care, long-term care, or related healthcare industries.
  • Outstanding communication and interpersonal skills with a knack for building lasting partnerships.
  • Goal-driven with strong organizational and problem-solving abilities.
  • Ability to travel regionally to support multiple Avantara facilities and foster relationships with community partners.
