You will help arrange medical appointments, medication management, and smooth transitions during discharges, transfers, and admissions.

You will maintain records, support care conferences, contribute to quality initiatives, and educate residents and families to participate actively in their care.

Responsibilities

About the Role:

• Hospital Liaison will serve as the primary point of contact between the community and healthcare providers, ensuring prompt and accurate communication regarding resident care needs and treatment plans.

• Collaborate with healthcare providers to coordinate medical appointments, medication management, and other healthcare-related services.

• Develop and maintain positive relationships with healthcare providers, ensuring consistent communication and cooperation to ensure the best possible outcomes for our residents.

• Hospital Liaison will also assist with the coordination of resident discharges, transfers, and admissions, ensuring a smooth transition for our residents.
