As a Hospital Liaison, you will play a vital role in ensuring seamless transitions between our assisted living facility and the hospital. Responsibilities:

• Coordinate with hospital staff to ensure efficient and effective communication regarding resident hospitalizations, discharges, and transfers

• Conduct thorough assessments of resident needs and develop personalized care plans in collaboration with our interdisciplinary team
