Garden Springs Healthcare is seeking a Hospital Liaison to support growth in the Greater Youngstown market. The role focuses on developing hospital referral relationships, increasing skilled nursing admissions, and expanding awareness of our post-acute programs.

The ideal candidate will have established hospital relationships in the region, a proactive mindset, and excellent communication and organizational skills.

Qualifications

• Minimum 3 years of experience in healthcare sales, hospital liaison services, or related field preferred.
• Established relationships within the Greater Youngstown hospital market strongly preferred.
• Experience in skilled nursing, post-acute care, or complex medical services desirable.
• Strong understanding of discharge processes, insurance authorizations, and post-acute placement.

Position Summary

Garden Springs Healthcare is seeking an experienced, relationship-driven Hospital Liaison to support our continued growth throughout the Greater Youngstown market. This position will play a key role in developing and maintaining hospital referral relationships, increasing skilled nursing admissions, and expanding awareness of our specialized clinical programs.

The ideal candidate has established relationships with hospital case managers, discharge planners, social workers, and healthcare providers throughout the region. We are looking for a motivated individual who takes ownership of their accounts, thrives in a fast-paced environment, and understands the importance of timely communication and exceptional customer service.

Key Responsibilities

Hospital Relationship Development

• Develop and maintain strong relationships with hospital discharge planners, case managers, social workers, physicians, and other referral partners throughout the Greater Youngstown market.

• Serve as the primary point of contact for assigned hospital accounts, building trust and positioning Garden Springs as a preferred post-acute care provider.

• Conduct regular hospital visits, educational presentations, and relationship-building activities.

• Identify new referral opportunities and develop strategies to increase market share.

Referral Management & Admissions

• Take ownership of the referral process, from initial referral through successful admission.

• Collaborate closely with our Centralized Admissions & Intake team, facility leadership, and clinical teams to ensure timely referral reviews and admission decisions.

• Coordinate with hospital discharge planners and facility teams to facilitate smooth patient transitions, including transportation, authorization updates, and discharge planning.

• Maintain proactive communication with referral sources, patients, and families throughout the admission process.

• Provide timely follow-up on pending referrals and address barriers that may delay admissions.

• Participate in weekend coverage and admission coordination as needed, consistent with liaison team expectations.

Business Development & Specialty Program Growth

• Drive skilled nursing admissions and census growth across assigned Garden Springs facilities.

• Promote our specialized clinical capabilities, including ventilator care, respiratory services, tracheostomy management, complex medical care, and other specialty programs.

• Identify opportunities to expand hospital partnerships and develop new referral pathways.

• Monitor referral trends, competitive activity, and opportunities for service-line growth within the Youngstown market.

• Collaborate with facility and corporate leadership to develop and execute market-specific growth strategies.

Performance & Accountability

• Meet or exceed established referral, admission, and business development goals.

• Track referral activity, conversion rates, lost referrals, and account performance.

• Maintain consistent communication with corporate leadership regarding market opportunities, challenges, and growth initiatives.

• Work collaboratively with facility liaisons, administrators, directors of nursing, and centralized admissions to maximize referral conversion.

Qualifications

• Minimum of 3 years of experience in healthcare sales, hospital liaison services, skilled nursing business development, discharge planning, or a related field preferred.

• Established relationships within the Greater Youngstown hospital market strongly preferred.

• Experience in skilled nursing, post-acute care, rehabilitation, or complex medical services highly desirable.

• Strong understanding of hospital discharge processes, insurance authorizations, and post-acute placement.

• Excellent interpersonal, communication, and organizational skills.

• Demonstrated ability to independently manage referral accounts and achieve admission goals.

• Ability to work collaboratively with clinical and operational teams.

• Reliable transportation and willingness to travel throughout the assigned market.
