Job summary

Ravenwood Health is seeking a Hospital Liaison in Ohio to support youth transitions from inpatient psychiatric care to community services. You will work closely with hospital social workers and CME staff to coordinate referrals and ensure safe, timely discharge planning.

The role requires building strong hospital partnerships, maintaining up-to-date documentation, and providing ongoing education to hospital partners about CME services and community resources.

Qualifications

  • Bachelor or Master’s degree plus 1 year of relevant experience.
  • LSW/LPC licensure in the state of Ohio.
  • Valid Ohio driver’s license with insurable driving record.
  • Ability to pass FBI/BCI background check.

Responsibilities

  • Serve as the primary liaison between inpatient psychiatric hospitals and the CME to ensure continuity of care for youth transitioning from higher levels of psychiatric care to community-based services.
  • Develop relationships with local hospital systems to support referrals and discharge planning and visit hospital departments at least monthly.
  • Designate a direct point-of-contact within hospital social work or discharge planning to bypass general intake or referral processes.
  • Maintain presence within inpatient units to monitor admissions, treatment planning, and discharges in real time.
  • Coordinate warm handoffs among inpatient, CME, outpatient providers, youth, and families within 72 hours post-discharge window.

Key Responsibilities
  • Serve as the primary liaison between inpatient psychiatric hospitals and the Care Management Entity (CME) to ensure continuity of care for youth transitioning from higher levels of psychiatric care to community-based services.
  • Establish relationships with local hospital systems to better support referrals and discharge planning. Through this relationship development work to visit local hospital departments at minimum once per month to further relationship and streamlining of referrals.
  • Designate and sustain a direct point-of-contact within each hospital’s social work or discharge planning department to bypass general intake or referral processes.
  • Maintain a consistent physical and/or virtual presence within inpatient psychiatric units to ensure real-time awareness of admissions, treatment planning, and anticipated discharges.
  • Develop and maintain collaborative relationships with hospital social workers, discharge planners, nursing staff, psychiatrists, and community providers.
  • Participate in psychiatric discharge planning to ensure CME prior to discharge, with emphasis on safe and timely transitions.
  • Coordinate warm handoffs among inpatient providers, CME staff, outpatient providers, youth, and families by working diligently to protect the 72-hour post discharge window by monitoring service linkage, medication access, and follow-up appointments.
  • Identify barriers to discharge or stabilization and present CME‑aligned solutions to hospital teams.
  • Manually enter real‑time admission and discharge data into CME tracking systems when CliniSync notifications are unavailable or delayed.
  • Provide education to hospital partners regarding CME services and community-based resources.
  • Maintain accurate, timely, and confidential documentation in accordance with Ohio Administrative Code, Medicaid requirements, CME standards, and agency policies.
  • Maintain effective communication for all cases assigned with all partners involved through staffing, weekly rounds, or child and family team meetings.
  • Assist to track statistics and outcomes related to transition coordination, discharge involvement, and post‑discharge continuity.
  • Attend staff meetings, supervision, and required trainings.
  • Demonstrate cultural competence and trauma‑informed practice when working with youth, families, and professionals.
  • Maintain confidentiality and professionalism in accordance with agency policy.
  • Adhere to the organization’s Mission, Vision, and Values.
  • Perform all other duties as assigned by Administration.
Knowledge, Skills, and Abilities
  • Experience working with youth with serious emotional disturbance, psychiatric conditions, or complex behavioral health needs.
  • Demonstrated ability to navigate hospital systems and collaborate with multidisciplinary clinical teams.
  • Strong relationship‑building, communication, and organizational skills.
  • High emotional intelligence and the ability to build trust with hospital staff working in high‑pressure environments.
  • Proactive problem‑solving skills, including the ability to identify system barriers and present practical solutions.
  • Ability to work independently while maintaining alignment with CME supervision and team structures.
  • Ability to utilize electronic health records, tracking systems, and agency‑issued technology.
Required Education and Experience
  • Bachelor or Master’s degree plus 1 year of relevant experience.
  • LSW/LPC licensure in the state of Ohio
  • Must possess valid Ohio driver's license and a driving record that does not preclude the employee from being covered by Agency liability insurance.
  • Must be able to pass an FBI/BCI background check
Preferred Education and Experience
  • Experience working within hospital, psychiatric, behavioral health, or care management systems preferred.
  • Knowledge of Ohio behavioral health systems, Medicaid services, and community-based resources preferred.
