Job summary

Hallmark Health Consulting Services is seeking a Hospital Liaison to act as the primary contact between our facility and local hospitals, physicians, and healthcare organizations. You will coordinate referrals, admissions, and patient transitions to ensure high-quality care and smooth transitions.

In this role, you will cultivate relationships with discharge planners and clinicians, promote our services to referral sources, and coordinate bed availability and placement while supporting families

Qualifications

• Experience in healthcare marketing, admissions, or case management preferred.
• Knowledge of skilled nursing, rehab, or long-term care services.
• Strong interpersonal and communication abilities.
• Ability to build and maintain professional relationships.
• Self-motivated with excellent organizational skills.
