Job summary

Medrina, LLC is seeking a Hospital Discharge Coordinator to join our hybrid team. The role coordinates discharge planning with hospital staff and post-acute providers, ensuring timely and safe transitions for patients in the Palm Beach, FL region.

The position requires an RN/LPN/LCSW/BSW/MSW with 3+ years in hospital case management, and comprehensive knowledge of Medicare/Medicaid. The role offers a $85,000 annual salary, telework stipend, and a robust benefits package with healthcare, 401(k)

Qualifications

RN/LPN/LCSW/BSW/MSW with hospital case management experience.
Minimum 3 years in hospital case management, discharge planning, utilization management, care coordination, or transitions of care.
Strong knowledge of Medicare, Medicare Advantage, Medicaid, and commercial insurance authorization processes.
Understanding of skilled nursing facility admission criteria and post-acute care resources.
Excellent communication, relationship-building, and organizational skills.
Ability to manage multiple priorities in a fast-paced hospital environment.

Responsibilities

Coordinate hospital discharge planning with case managers and clinical teams.
Identify appropriate post-acute care options and coordinate transitions.
Maintain referrals, bed availability, and placement options with partner facilities.
Document care coordination activities and track KPIs.
Ensure HIPAA, CMS compliance and accurate reporting.
