Qualifications

• Licensed Pharmacist with active license (IDFPR).
• Bachelor's Degree in Pharmacy required.

Responsibilities

• Accepts responsibility for checking all medications leaving the department to verify quality assurance.
• Knowledge of chemotherapy agents and hyperalimentation.
• Knowledge of IV admixture and narcotic control.
• Knowledge of investigational drugs and therapeutic drug monitoring.
• Accurate entry of orders into the computer and ensure current medication administration reports.
• Communicate with healthcare professionals regarding orders, drug interactions, allergies and dosing questions.
• Work with healthcare professionals to provide cost-effective drug therapy and use pharmacy systems for therapy decisions.
• Communicate medication orders and discuss issues with nurses or physicians as needed.
• Participate in planning, updating services and in-service programs within the department.
• Work with various computer systems, write in patient charts, and dose medications per patient parameters.
