Northeast Georgia Health System in Gainesville, FL seeks a Case Management Social Worker to provide hospital-based medical social work services, assessing social, psychological, cultural, environmental and financial needs and coordinating discharge planning.

You will collaborate with the care team, use crisis intervention and SBIRT as needed, support families, and ensure appropriate referrals to community resources across age groups from neonate to geriatric.

Formación

• Masters degree in Social Work from an accredited program; licensure preferred.
• Experience: at least 1 year in hospital or related health care setting.
• CCM or ACM certification preferred.

Responsabilidades

• Assess patient social, psychological, cultural, environmental and financial needs to inform discharge plans.
• Coordinate referrals for post-acute care facilities (NH/SNF/AL) as needed.
• Collaborate with the care team, patients and families to implement care plans.
• Provide crisis intervention and advocacy to support patient outcomes.
• Complete appropriate documentation and ensure compliance with NASW Code of Ethics.
• Respond to referrals/requests within 24 hours and participate in rounds.
