Northeast Georgia Health System is seeking a Case Management Social Worker to provide medical social work services to patients in a hospital setting. The role assesses social, psychological, cultural, environmental, and financial needs and collaborates with the care team to develop discharge plans.

The position requires crisis intervention, advocacy, and cross-coverage across settings, including weekend rotations.

Competenze

• Master's Degree in Social Work from an accredited program; licensure preferred.
• Minimum of 1 year experience in hospital or related health care setting.
• CCM or ACM certification preferred for advanced case management.

Mansioni

• Gathers information on patients' social, psychological, cultural, environmental, and financial factors to assess discharge needs.
• Uses SBIRT techniques to identify at-risk alcohol and drug use and arrange referrals as needed.
• Expedites discharge planning through networking and teamwork to support patient stay and transition.
• Coordinates placement (NH, SNF, AL, LTAC, Acute Rehab) and transportation post-discharge; verifies insurance and signatures.
• Maintains patient advocacy and ensures Freedom of Choice and Medicare communications.
