Job summary

Gonzaba Medical Group in San Antonio, TX is hiring a Hospital Case Manager (RN). This role focuses on concurrent review of GMG MC patients in hospital and facility settings, coordinating discharge plans with physicians, patients, and families to ensure safe, timely transitions.

The ideal candidate has 3–5 years of RN experience in acute care, strong knowledge of utilization management, and proficiency with EMR systems. Bilingual English/Spanish is preferred to support diverse communities.

Qualifications

• Active, unrestricted RN license (Texas)
• 3–5 years RN experience; hospital/acute care
• Experience in care coordination and discharge planning

Responsibilities

• Orient all newly enrolled Managed Care patients to Gonzaba Medical Group.
• Round daily on GMG MC Patients in the hospital setting, and at least weekly on those in the SNF setting.
• Assess medical, physical, psycho-social and transition needs to anticipate DC services and follow-up.
• Confer with physician(s), patient, and families in developing discharge plans.
• Enter timely and accurate information into the EMR documenting communications, referrals and plans.
• Collaborate with GMG CM team to communicate coverage determinations and authorizations post DC.
• Coordinate with MC Medical Director on patient status and issues.
• Serve as clinical liaison with hospital, clinical and administrative staff.
