Responsibilities

• Utilizes multiple sources to capture an accurate daily Hospital Census for Secure Horizon / Evercare patients in hospital, SNF, and outpatient settings.
• Distributes census per protocol to physician and staff.
• Contacts patients/families post-discharge to schedule timely follow-up appointments.
• Communicates patient concerns to Case Manager nurses for prompt attention and follow-up.
• Schedules patients for High-Risk Team visits based on audits and needs.
• Assists with patient transportation arrangements and related coordination.
