Job summary

Gonzaba Medical Group inSan Antonio, TX is seeking a dedicated RN Case Manager to coordinate care for high‑risk patients and guide them through complex treatment plans alongside GMG providers and services.

You will perform triage, manage documentation in the EMR, and collaborate with PCPs, nurses, and social workers to prevent unnecessary hospital visits. Requires BSN and active RN license. This is a full-time clinic position.

Qualifications

• RN Case Manager experience or similar in a clinic
• Excellent written and verbal communication skills
• Knowledge of medical terminology and care coordination best practices
• Experience with EMR systems and patient medi­cation management

Responsibilities

• Direct patients to appropriate level of care based on history and provider advice.
• Maintain high-risk patient database and review census weekly.
• Coordinate care across GMG providers, PCPs, and home health services.
• Assist with discharge planning and follow-up to prevent readmissions.
• Document care plans and patient interactions accurately in EMR.
• Educate patients on disease processes, treatments, and follow-up steps.
