Gonzaba Medical Group seeks a Hospital Case Manager Assistant to support the Hospital Case Management Nurses by monitoring census, scheduling follow-ups, and coordinating referrals. The role involves accurate data entry into the EMR, assisting with authorizations, and maintaining high-quality customer service with patients and providers.

Candidate should be proficient in English; bilingual Spanish preferred.

Qualifications

• Minimum high school education or equivalent required.
• One year experience in a clinic, doctor's office or hospital preferred.
• Bilingual English/Spanish fluency preferred.

Responsibilities

• Composes daily Hospital Census for Humana, Wellpoint and SCAN across hospital, SNF and rehab facilities.
• Verify insurance eligibility and benefits via HMO lists or by phone with insurers.
• Assist Hospital case managers in obtaining authorizations from the health plan.
• Prints inpatient authorization/denial letters and mails to patients and faxes to providers.
• Schedules patients to the High-Risk Team visits as requested by HCM.
• Enter timely, accurate information into the EMR to document communications and plans.
• Assist with producing letters, faxing documents and mail outs.
• Maintain current knowledge of health plan benefits and networks.
