Job summary

Witham Health Services in Lebanon, IN is seeking a licensed Physical Therapist to plan and perform high-quality therapy for patients referred to the Witham Memorial Hospital Physical Therapy Department and its contractual clients.

The ideal candidate holds a Doctorate/Master's/Bachelor's in Physical Therapy, active Indiana license, and CPR (BLS Course C). They must complete in-services and be familiar with disaster and safety procedures. The position is full-time, days, M-F, 8:00a-4:30p.

Qualifications

• Bachelor's, Master's, or Doctorate degree from an accredited Physical Therapy Program.
• Current licensure in Indiana or temporary licensure for new graduates.
• Mandatory inservices upon employment and yearly thereafter.
• Knowledge of disaster and safety procedures.
• Review Rehab Services Department policy every two years.
• CPR Basic Life Support, Course C.

Responsibilities

• Plan and perform high-quality physical therapy for referred patients.
• Comply with Indiana state licensing and hospital safety procedures.
• Maintain CPR certification and participate in in-services.
