Job summary

Children’s Wisconsin seeks a Social Worker (PRN) to address the social, psychological and economic needs of children and families in inpatient and outpatient settings. This role focuses on discharge planning and referrals to community resources to ensure follow-through on treatment plans.

The onsite position is 2–3 days per week from 8:00 am to 4:30 pm, with no evenings or weekends. Master’s in Social Work and Wisconsin licensure are required; hospital experience is preferred, and supervision

Qualifications

• 1+ years of social work or related field experience required.
• Hospital experience preferred.
• Master’s Degree MSW required.

Responsibilities

• Assists families with short term, crisis oriented counseling; referrals to community agencies, programs and services.
• Educates patients/families about illness, hospital procedures and community resources.
• Helps plan for the future health care needs of the child whether or not admitted to the hospital.
• Consults with other professional staff to assure understanding of strengths/needs of patients and families.
• Performs duties in courteous and helpful manner with respect towards patients, families, staff and visitors.
