Job summary

UW Health's American Family Children's Hospital in Madison, WI is seeking a qualified Master’s Level Social Worker to join our team. This full-time role emphasizes patient- and family-centered care within a values-driven hospital environment.

You will evaluate complex psychosocial scenarios, collaborate on discharge planning, and support families through transitions and rights-based care, leveraging crisis intervention when needed.

Qualifications

• Master's Degree in Social Work required.
• CAPSW Wisconsin within 90 days required.
• CISW Wisconsin within 90 days required.
• LCSW Wisconsin within 90 days required.

Responsibilities

• Evaluate complex patient and family situations with psychosocial components.
• Collaboratively develop patient goals, monitor progress, address barriers to goal achievement and evaluate outcomes of individualized care plans.
• Consider barriers including coping with chronic illness, mental health history/symptoms, cognitive development, adverse developmental events, resistance to treatment, health system impediments, lack of social support and vulnerability.
• Participate in discharge planning for patients requiring post-discharge services including transitions to skilled nursing facilities, assisted living facilities, outpatient dialysis, Acute Rehab, hospice and transportation needs.
• Engage patient/family in the treatment and decision-making process with respect to patient rights, confidentiality, privacy, and right to self-determination.
• May utilize crisis intervention, conflict resolution, and mediation to address patient/family concerns.
