Job summary

Children’s Wisconsin is seeking a PRN Social Worker to support inpatient and outpatient families in Milwaukee. You will assist with discharge planning, referrals to community resources, and coordinating care to ensure families understand the treatment plan.

The role requires a Master’s Degree in Social Work (MSW) and 1+ year of hospital social work experience, with no evenings or weekends and a schedule of 8:00 am–4:30 pm on days as needed. PRN coverage based on team needs.

Qualifications

• Hospital experience preferred.

Responsibilities

• Understands and addresses the social, psychological and economic needs of patients and families in inpatient and outpatient settings.

The Social Worker will perform a variety of duties to address the social, psychological and economic needs of children and their families, in both an inpatient and outpatient setting. The person in this role is concerned with continuity of care which usually involves discharge planning for children who are inpatients and their families to ensure they understand and follow the recommended treatment plan, and referrals to community agencies and programs.

Location and Availability

• Location: Onsite - Milwaukee Hospital
• PRN: 2 days possibly 3 days per week (based on needs of the team)
• Provide coverage for teammates who are out of the office
• Work in a variety of areas within the hospital
• Hours: 8:00 am - 4:30 pm
• No evenings
• No weekends

Experience

• 1+ years of previous social work or related field experience required
• Hospital experience preferred

Education

• Master's Degree MSW required

Licenses and Certifications

• One of the following:
• License Social Worker Advance Practice - APSW - (121) - State of Wisconsin
• License Social Worker Clinical (123) - State of Wisconsin
• License Social Worker Independent (122) - State of Wisconsin

Essential Functions

• Understands and satisfies the needs of the neonatal, infant, toddler, pre‑school, school‑age, adolescent and/or adult patient in regards to their growth and development process.
• Assists families in meeting their needs through: Short term, crisis oriented counseling; referrals to community agencies, programs and services; educating patients/families about illness, hospital procedures and community resources.
• Helps plan for the future health care needs of the child whether or not admitted to the hospital.
• Consults with other professional staff to assure that they understand the strengths and needs of patients and families. Assesses the medical care/history of the child and family to assist medical staff with diagnosis and treatment plan.
• Performs duties in courteous and helpful manner with respect towards patients/families, department staff, visitors and employees from other departments.

Please Note: This role does not provide supervision for clinical hours toward licensure.
